Registration

To help us prepare for your appointment, we need to get some important information about you! You can provide us this information in one of several ways:

Paper Copy

You should receive a packet of information in the mail shortly after your appointment is scheduled. In that packet contains a paper copy of our registration paperwork. Please fill out the forms and provide them to us at your appointment. If you need another copy, you can download the forms here!

You can also submit your information on a secure online form available at the Midwest Eye Institute website. Your information will be encrypted and made available to our scheduling staff.
